Regarding the Blue Note/ESP question, I wonder if Blue Note recorded the concert and then decided not to release it, giving the rights back to Coleman? It wouldn't be the first time Blue Note didn't release a full record (until discovered by Michael Cuscuna decades later).

Since this was at the time that Ornette was rejecting many dates because the compensation was insufficient (in his eyes), I wonder if the royalty rate was not acceptable to him.
Of course he did release albums on Blue Note after this...

Ok this is closer to your wondering, LG, taken from the very informative essay at the Canary Records bandcamp page for Chappaqua Suite: "Both Blue Note and Impulse Records ultimately passed on releasing the tape of the concert. In need of money, Coleman handed the tape and the rights in exchange for a flat fee to Bernard Stollman whose ESP-Disk label released it in 1965, and has reissued it many times since. Coleman rarely, if, ever mentioned the LP’s existence, and when he was asked about Stollman’s business practices later, Coleman called him an “assassin.” It included his first composition for strings, a through-composed nine-minute quartet titled “Dedication to Poets and Writers.” "

The problem with that theory (ignoring your later information) is that Blue Note surely had an exclusive right to recording the concert and so would have sued the hell out of ESP if they jumped the gun. Anyway, it's great that the full concert is now out in the ether!
